Apesar da sua comprovada eficácia na redução da mortalidade, o seu impacto na qualidade de vida é ainda fonte de alguma discussão, sendo um tema merecedor de investigação detalhada.
Objectivos: Avaliar a qualidade de vida em doentes com insuficiência cardíaca antes e após a implantação de cardioversor desfibrilhador implantável associado ou não a sistema de ressincronização biventricular (BIV).
Metodologia: Estudo prospectivo comparando características demográficas, clínicas e qualidade de vida em 40 doentes, distribuídos por dois grupos, portadores de cardioversor desfibrilhador implantável e portadores de cardioversor desfibrilhador implantável associado a sistema de ressincronização biventricular. A avaliação da qualidade de vida foi efectuada através do World Health Organization Quality of Life Questionnaire.
Resultados: Nos grupos estudados observou-se uma tendência para o aumento da qualidade de vida ao fim de três meses. Contudo, após um ano, o grupo CDI apresentou scores inferiores no domínio social, ao contrário do grupo que associa BIV e CDI, que registou um aumento dos scores no domínio físico.
Conclusão: No âmbito geral, a implantação de cardioversores desfibrilhadores implantados alteram a definição individual de qualidade de vida. Tendo em conta que a condição de base do doente com insuficiência cardíaca apresenta scores médios baixos de qualidade de vida, é imprescindível fornecer ao doente as devidas informações acerca do impacto variável dos CDI. Background: The implantable cardioverter defibrillator (ICD) is a life-saving device for patients with ventricular arrhythmias, which represent a constant threat to life. Despite its proved efficiency on the reduction of mortality, its impact on the quality of life is still source of some discussion, being an issue deserving further detailed research. Objectives: Evaluating the quality of life in patients with congestive heart failure (CHF) before and after the implantable cardioverter-defibrillator implantation associated or not to a cardiac resynchronization therapy (CRT). Methodology: Prospective study comparing demographic, clinical and quality of life characteristics in 40 patients, distributed in two groups, implantable cardioverter-defibrillator carriers and implantable cardioverter-defibrillator carriers associated to cardiac resynchronization therapy. The evaluation on the quality of life was done through the World Health Organization Quality of Life Questionnaire. Results: The studied groups showed an increase of quality of life after three months. However, after one year, the ICD group presented inferior scores on the social domain, contrary to the group that associates CRT and ICD, which registered an increase on the physical domain scores. Conclusion: In general, the implantable cardioverter-defibrillator implantation alters the individual definition of the quality of life. Given that the base condition of the patient with cardiac insufficiency presents low average scores of quality of life, it is unquestionable to provide the patient with the right information about the variable impact of ICD. |
