O papel da lavagem broncoalveolar (LBA) na patologia pulmonar intersticial associada a doenças do tecido conjuntivo (DPI-DTC) mantém-se desconhecido e não é recomendado de forma rotineira nestes casos.
Objetivo
Avaliação do estudo da lavagem broncoalveolar em doentes com DPI-DTC e a sua associação com os parâmetros radiológicos e funcionais.
Métodos
Estudo retrospetivo incluindo doentes com doença pulmonar instersticial - doença do tecido conjuntivo (DPI-DTC) associados a Esclerose Sistémica Progressica (ESP), Artrite Reumatóide (AR), polimiosite ou dermatomiosite (PM/DM), Lúpus Eritematoso Sistémico (LES) e Síndrome de Sjögren (SS), referenciados para realização de lavagem broncoalveolar.
Resultados
Foram incluídos 41 doentes, incluindo 16 com ESP, 7 com PM/DM, 10 com AR, 5 com LES e 3 com SS. Os grupos com AR e PM/DM demonstraram percentagens médias de linfócitos e neutrófilos elevadas, enquanto os com ESP e SS apresentavam predominantemente neutrofilia. Observou-se uma tendência para a elevação do rácio CD4/CD8 nos pacientes com LES. A neutrofilia e eosinofilia associam-se a valores de VEMS e CVF diminuídos respetivamente. O padrão de fibrose na TCAR correlacionou-se com uma CVF, VEMS e DLCO diminuídos.
Conclusões
Linfocitose e neutrofilia associam-se à AR e PM/DM, enquanto neutrofilia é o achado predominante na ESP e SSj. O padrão de fibrose na TCAR, a neutrofilia e a eosinofilia associam-se a uma pioria dos parâmetros das provas de função respiratória (PFR). São necessários mais estudos no sentido de avaliar utilidade do LBA no diagnóstico diferencial e prognóstico nos casos de DPI-DTC. Background The role of bronchoalveolar lavage (BAL) in interstitial lung disease associated with connective tissue disease (CTD-ILD) remains unknown and is not routinely recommended. Aim Evaluation of BAL results in patients with CTD-ILD and its radiological and functional correlates. Methods Retrospective study including patients with (CTD-ILD) referred for bronchoalveolar lavage in a tertiary hospital. Results 41 patients were found, including 16 with ESP, 7 with PM/DM, 10 with RA, 5 with SLE and 3 with SSj. RA and PM / DM groups showed an elevated lymphocyte and neutrophil percentage, while the PSS and SSJ groups had predominantly high neutrophils. There was a trend towards a higher CD4/CD8 ratio in patients with SLE. BAL fluid neutrophilia in PSS patients appears to be associated with a decreased diffusion capacity. Neutrophilia and eosinophilia are associated with decreased FEV1 and FVC respectively. HRCT showing mainly fibrosis correlated strongly with a decreased FVC, FEV1 and DLCO. Conclusions BAL lymphocytosis and neutrophilia are associated with RA and PM/DM, whereas neutrophilia is the predominant finding in ESP and SSj. HRCT fibrosis and BAL neutrophilia and eosinophilia are associated with worse lung function. Further studies are needed to define the usefulness of bronchoalveolar lavage in the differential diagnosis and prognosis in cases of ILD-CTD. |
