mais de 30 milhões de pessoas. Atendendo ao aumento da esperança média de vida, é
espectável que o peso económico e social desta doença venha a aumentar drasticamente.
A patogénese da doença tem sido associada à ocorrência de micro derrames bem como à
agregação, deposição, modificação e acumulação de peptídeos Beta Amiloides (Aβ) e
fosforilação e agregação da proteína Tau no cérebro. Assim, placas extracelulares, contendo
várias formas de Aβ e nós neurofibrilares intracelulares, constituídos por proteínas Tau
hiperfosforiladas, são os dois maiores marcadores patológicos da DA. A hipótese da cascata
amiloide refere que a deposição Aβ é o evento inicial na patogénese da doença e que
precede o aparecimento dos sintomas em largos anos.
Atualmente, não existe cura ou maneira de prevenir a doença; as terapêuticas disponíveis
apenas dão benefícios sintomáticos temporários. Novas abordagens imunoterapêuticas têm
demonstrado ser relativamente eficazes na redução da acumulação Aβ, indicando que a
intervenção é mais eficaz nas fases iniciais da doença, realçando a importância de um
diagnóstico o mais precocemente possível.
A terapêutica Aβ com imunização ativa e passiva tem um potencial elevado para ser eficaz na
remoção destes peptídeos no cérebro, podendo, assim, retardar a progressão da patologia.
Contudo, atingir resultados eficazes e seguros tem sido um grande desafio. Esforços estão a
ser feitos continuamente para tratar indivíduos com risco de DA, antes ou nas fases iniciais
do declínio cognitivo, com a esperança de prevenir ou atrasar a progressão da doença. Alzheimer disease (AD) is the most common cause of dementia, afflicting more than 30 million people worldwide and, with the increase of life expectancy, the social and economic burden from this disease will increase drastically. The pathogenesis of the disease has been associated with the occurrence of micro-strokes in brain vasculature and with aggregation, deposition, modification and accumulation of amyloid beta peptides (Aβ) in the brain, as well as phosphorylation and aggregation of Tau protein. Thus, extracellular plaques containing various forms of Aβ and intracellular neurofibrillary tangles, composed of hyperphosphorylated Tau are the two major pathological hallmarks of the AD. The amyloid cascade hypothesis states that Aβ deposition is the initial event in the pathogenesis of the disease and that it precedes the symptoms in many years. Currently, there is no cure or way to prevent the disease; available therapies provide only temporary symptomatic benefits. However, novel immunotherapeutic approaches have been proven effective in reducing the accumulation of Aβ. These interventions were shown to be more effective in the early stages of the disease, enhancing the importance of an early diagnosis. Aβ therapy by active and passive immunization has a high potential to be effective in the removal of these peptides in the brain, and may, therefore, slow the progression of the disease. However, achieving effective and safe results has been challenging. Efforts are continuously being made to treat individuals at risk for AD, before or at the earliest stages of cognitive decline with the hope of preventing or delaying disease progression. |
