postural, sendo o mais importante e comum a doença de Parkinson.
A Doença de Parkinson foi descrita por James Parkinson em "An Essay on the Shaking
Pulse" (1817). É um dos distúrbios do movimento que mais acomete os idosos, embora
também possa afectar doentes jovens.
Caracteriza-se por uma desordem progressiva do movimento devido à disfunção dos
neurónios secretores de dopamina nos gânglios da base, que controlam e ajustam a
transmissão dos comandos conscientes vindos do córtex cerebral para os músculos do corpo
humano.
A Medicina Nuclear segundo a Organização Mundial da Saúde define-se como: “A
especialidade que se ocupa do diagnóstico, tratamento e investigação médica mediante o uso
de radioisótopos como fontes radioactivas não seladas.”
Trata-se de uma especialidade médica que se fundamenta na utilização da energia nuclear
para fins médicos de diagnóstico e de terapia mediante o uso de isótopos radioactivos. Como
recurso diagnóstico, a Medicina Nuclear é um meio seguro e eficiente, em geral indolor e
minimamente invasivo, para se obter informações que, de outra maneira, seriam impossíveis
de conseguir. Abrange um conjunto de procedimentos de alta sensibilidade para encontrar
alterações na função dos órgãos estudados, com a virtude de identificar, precocemente,
numerosas alterações orgânicas e funcionais em relação a outros métodos diagnósticos. Para
esse fim são administrados radiofármacos, sendo a via endovenosa a mais frequente.
O diagnóstico da doença de Parkinson numa fase precoce pode ser difícil, mesmo em centros
especializados. Não existe um marcador biológico nomeadamente testes de laboratório ou
exames de imagem que até o momento sejam capazes de fazer o diagnóstico. Esta doença é
identificada essencialmente com base na observação clínica do doente. Sabe-se no entanto que a Medicina Nuclear através das suas técnicas, nomeadamente a
tomografia computadorizada por emissão de fotão único e a tomografia por emissão de
positrões, pode auxiliar no diagnóstico dos Síndromes Parkinsónicos e diagnóstico
diferencial.
Através destas técnicas é possível estudar diferentes aspectos da fisiologia cerebral que se
encontram alterados na Doença de Parkinson e outros Síndromes Parkinsónicos.
Nomeadamente, alterações no sistema dopaminérgico quer a nível pré sináptico, quer a nível
pós sináptico.
Destaca-se o estudo das alterações a nível do transportador da Dopamina. Para este fim o I123-
β-CIT, o I123-FP-CIT e o 99mTc-TRODAT são os principais radiofármacos utilizados.
Existe, inclusivamente, uma técnica legalmente aprovada a nível Europeu para diagnóstico
diferencial entre os Síndromes Parkinsónicos e o Tremor Essencial. Parkinsonian Syndromes are characterised by tremor, hypocynesy, rigidity and postural instability, being Parkinson’s Disease the most common and important one. Parkinson´s Disease was described by James Parkinson in “An Essay on the Shaking Pulse” (1817). It consists of one of the movement disorders which strikes elder people the most, although it can affect younger patients as well. It is characterised by a progressive movement disorder due to a dysfunction of the neurons responsible for the segregation of dopamine in base ganglions, which control and adjust the transmission of conscious actions stemming from the cerebral cortex and towards the muscles of the human body. According to the World Health Organization, Nuclear Medicine can be defined as “the medical specialty that is responsible for the diagnosis, the treatment and the medical investigation by means of use radioisotopes as unsealed radioactive sources”. This speciality is based on the use of nuclear energy for medical purposes of diagnose and therapy, using radioactive isotopes. As a diagnostic resource Nuclear Medicine is a safe, effective, generally painless and minimally invasive means to get information which would otherwise be impossible to obtain. It ranges from a number of procedures of high sensitivity in order to early identify alterations in the studied organs, with the added advantage, in comparison with other methods, of precocious identification of numerous organic and functional alterations. With this aim in mind, radiopharmaceuticals are administered, being the endovenous approach the most common one. In early stages Parkinson´s Disease diagnosis can prove quite difficult, even at specialised medical centres. For the time being there is no biological marker, either through laboratory tests or radiologic exams, with capability of diagnosis, being this disease most commonly identified through direct clinical observation of the patient. It is widely known, however, that Nuclear Medicine and its techniques, both single-photon emission computed tomography and positron emission tomography, can help diagnose Parkinsonian Diseases and differential diagnosis. Through these techniques it is possible to study the different aspects of the cerebral physiology which are modified in Parkinson´s Disease and in other Parkinsonian Syndromes, such as alterations in the dopaminergic system, both at pre-synaptic and pos-synaptic levels. The study of alterations in dopamine transporters should be highlighted. In this case the chief radiopharmaceuticals used are I123-β-CIT, I123-FP-CIT and 99MTc-TRODAT. Moreover, there is a legally approved technique in Europe to perform differential diagnosis between Parkinsonian Syndrome and Essential tremor. |
