As taxas de resposta a este tratamento rondam os 50%, mas apresentam uma variabilidade geográfica. Este estudo investiga se os pacientes com TI associada à infeção por Hp (TI-Hp+) da região Centro de Portugal beneficiam desta modalidade terapêutica.Metodologia: Foram estudados retrospetivamente 28 adultos com TI-Hp+, constantes dos registos do Serviço de Gastrenterologia do Centro Hospitalar e Universitário de Coimbra, em que se conseguiu eliminar a bactéria após tratamento com antimicrobianos. Determinou-se o impacto no aumento de plaquetas pelo menos 6 meses após a confirmação da erradicação.Resultados: Houve “Ausência de Resposta” na contagem de plaquetas em 16 doentes (57,1%), “Resposta Completa” em 11 doentes (39,3 %) e “Resposta Parcial” em 1 doente (3,6%). Observou-se maior aumento de plaquetas no grupo com “Resposta Completa” [(89,3±47,4)×10^9/L] em relação ao grupo com “Ausência de Resposta” [(-5,8±12,3)×10^9/L] (p=0,001). Também houve maior aumento de plaquetas no grupo com “Qualquer Resposta” (“Resposta Completa” + ”Resposta Parcial”) [(79,25±45,2)×10^9/L] em relação ao grupo com “Ausência de Resposta” (p=0,0001). Não foram encontradas variáveis preditivas da ocorrência de resposta favorável na contagem de plaquetas.Conclusão: Uma amostra significativa de doentes com TI-Hp+ da região Centro de Portugal teve um aumento importante de plaquetas em resposta à erradicação de Hp. A deteção e erradicação deste microrganismo deve ser considerada em doentes com TI. Introduction: The infection by Helicobacter pylori (HP) is a secondary cause of immune thrombocytopenia (IT), whose therapeutic approach includes the research of this bacteria and its eradication. The response rate to this treatment is around 50%, but it is linked with some geographic variability. This research investigates if the patients with IT associated with infection by Hp (IT-HP+) from de Centre region of Portugal would benefit from this therapeutic modality.Methods: we retrospectively studied 28 adults with IT-Hp+, who were part of the Gastroenterology service’s registries of Centro Hospitalar e Universitário de Coimbra, and in whom it was possible to eliminate the bacteria after antibiotic treatment. We determined the impact in the increase of platelet count at least 6 months after the eradication was confirmed.Results: There was “Absence of Response” in the platelet count of 16 patients (57,1%), “Complete Response” of 11 patients (39.3%) and “Partial Response” of 1 patient (3.6%). We observed a greater increase of platelets in the “Complete Response” group [(89,3±47,4)×10^9/L] than in the “Absence of Response” group [(-5,8±12,3)×10^9/L] (p=0,001). There was also a greater increase of platelets in the “Overall Response” group (“Complete Response” + “Partial Response”) [(79,25±45,2)×10^9/L] than in the “Absence of Response” group (p=0,0001). No predictive variables were found for a favourable response in the platelet count.Conclusion: A significant amount of patients with IT-Hp+ from the Centre region of Portugal has revealed an increase in the platelet count with Hp eradication. The diagnosis and eradication of this bacteria in patients with IT should be taken into account. |
