central. O processo degenerativo ocorre em consequência da destruição da bainha de
mielina por parte de células T auto-reativas e auto-anticorpos, causando interferência na
comunicação entre o cérebro, espinal medula e outras áreas do corpo. A etiologia da
doença não é ainda totalmente conhecida embora fatores genéticos e ambientais como por
exemplo infeções virais possam desempenhar um papel importante. A EM não tem cura
sendo o tratamento fundamentalmente direcionado para a diminuição dos sintomas e dos
episódios de recorrência.
Dado o carácter autoimune da patologia, novas abordagens terapêuticas têm-se focado na
modulação do sistema imune, nomeadamente na indução de tolerância a antigénios
relevantes na EM. Neste contexto, uma das estratégias mais promissoras passa pela indução
de tolerância através da utilização de células dendríticas (DCs) tolerogénicas (TolDCs). As
DCs são células apresentadoras de antigénios com uma capacidade inigualável de controlar
as respostas imunes, podendo induzir imunidade ou tolerância.
A utilização de TolDCs carregadas com antigénios mielínicos tem revelado excelentes
resultados em diversos modelos animais de EM. Nestes estudos pré clínicos as vacinas de
TolDCs mostraram ser seguras e induzir tolerância a auto-antigénios quer por indução de
células T reguladoras quer por inibição direta de células efetoras. Apesar dos resultados
serem extremamente promissores a transposição para a prática clínica tem sido bastante
lenta, sendo no entanto expectável que possa vir a ser implantada num futuro próximo com
um impacto significativo na melhoria da qualidade de vida dos doentes com EM.
A presente monografia visa desta forma transmitir uma perspetiva sobre o estado atual das
vacinas baseadas em DCs para a prevenção e tratamento da EM. Multiple Sclerosis (MS) is a chronic autoimmune disease that affects the central nervous system. The degenerative process occurs due to the destruction of the myelin sheath by auto reactive T cells and auto antibodies. This causes interferences in the communication between the brain, spinal cord and other body areas. The disease’s etiology is not yet fully known although genetic and environmental factors, such as viral infections, may play an important role. MS has no cure and the treatment is mainly directed to the reduction of symptoms and recurrent episodes. In view of the autoimmune nature of the pathology, new therapeutic approaches have focused on the modulation of the immune system, namely in the induction of tolerance to the relevant antigens in MS. In this context, one of the most promising strategies involves the induction of tolerance by using tolerogenic dendritic cells (TolDCs). DCs are antigenpresenting cells with an unrivalled ability to control immune responses by inducing immunity or tolerance. The use of TolDCs loaded with myelin antigens has showed excellent results in several animal models of MS. In these preclinical studies, TolDCs vaccines showed to be safe and to induce tolerance to auto antigens by induction of regulatory T cells or by direct inhibition of effector cells. Despite the extremely promising results, the transposition into clinical practice has been rather slow. However, it is expected that it may be deployed in the near future with a significant impact on improving the quality of life of MS patients. This monograph intends to convey an overview of the current state of DC based vaccines for the prevention and treatment of MS. |
