Neste artigo serão descritos os aspetos fundamentais relativos ao seu diagnóstico e epidemiologia. Serão também apresentados os fatores de risco para a sua etiologia como são os fatores genéticos e de temperamento, fatores cognitivos e comportamentais e fatores ambientais, cujos diferentes caminhos e combinações poderão conduzir ao desenvolvimento desta patologia. De seguida serão apresentados três modelos (Clark & Wells, Rapee & Heimberg, e Hofmann) que pretendem descrever a geração e manutenção desta patologia, e que são utilizados na prática clínica como suporte das terapias cognitivo-comportamentais. Os processos habitualmente associados à manutenção desta patologia, nomeadamente o papel do processamento antecipatório, dos comportamentos mal-adaptativos de segurança e de evitamento, da utilização de atenção auto-focada e do processamento pós-evento serão também analisados. As terapêuticas psicológicas e farmacológicas atualmente utilizadas no tratamento da PAS serão então apresentadas. Nas terapêuticas psicológicas serão descritas várias das intervenções que têm sido administradas, com particular enfase para a TCC individual, que tem vindo a ser apontada como terapêutica de 1ª linha. Seguidamente será apresentada a terapêutica farmacológica considerada eficaz no tratamento da PAS, com especial enfase nos SSRIs, SNRIs, e MAOIs. Em ambas as terapêuticas serão apresentadas meta-análises que realizam comparações entre a eficácia das diferentes intervenções, e introduzidas as guidelines da NICE no tratamento desta patologia. Social Anxiety Disorder (SAD) is a disorder with a high prevalence in the western population, been characterized by an exaggerated and persistent fear regarding performance or social situations, that interferes with daily activity. This article will start by describing the fundamental aspects related to its diagnosis and epidemiology. Risk factors for its etiology will also be presented, such as genetic, temperamental, cognitive, behavioral and environmental factors, whose different paths and combinations may lead to the development of this pathology. Following, three models (Clark & Wells, Rapee & Heimberg, and Hofmann), which are intended to describe the generation and maintenance of this pathology, will be presented. These models are also used to support the cognitive-behavioral therapies in the clinical practice. The processes usually associated with the maintenance of this pathology, namely the role of anticipatory processing, safety-behaviors, avoidance, self-focused attention and post-event processing will also be analyzed. The psychological and pharmacological therapies currently used in the treatment of SAD will then be described. Regarding the psychological therapies, several of the interventions that have been administered will be presented, with particular emphasis to the individual CBT, which has been recognized as the gold standard therapy. Next, the most effective pharmacological therapies in the treatment of SAD will be analyzed, with special emphasis to SSRIs, SNRIs, and MAOIs. In both approaches, meta-analyzes that make comparisons between the effectiveness of the different interventions will be presented, and the NICE guidelines for the treatment of this pathology will be introduced. |
